How To Fill Out A Form Online Using Adobe Reader

Step 1. Download the form in Adobe Reader.

Open with Adobe Acrobat Reader DC

Step 2. Select the Fill & Sign tool and be sure and complete the entire form.

Click on Fill and Sign

Step 3. Add your signature. Select "Sign" from the top tool bar. (Here you can add a signature.)

Step 4. Draw your signature with a mouse or trackpad. Then you can place the signature on the form and scale it to the proper size.
